Long Term Conditions - Medlink
We now use an online system to review long-term health conditions and medicines. When it’s time for your review, you’ll receive a link via email, text message or NHS App.
Please click or tap the link to begin. You can use a smartphone, tablet, or computer. You can also copy the link into a browser on a larger screen.
If a questionnaire is required, you will be sent a personalised review combining common conditions into one annual check.
Questionnaire answers are saved automatically. If you don’t finish in one go, please return to the link later. Feel free to ask a friend or family member to help.
You may also be asked to book an appointment (eg blood test) or provide a sample (eg urine test). Follow the instructions to ensure we have everything needed to care for you. Please note that even if you indicate that you prefer to ‘complete a review without further discussion’ within a questionnaire, you still need to arrange any tests or appointments as instructed.
Once all requirements are complete, the practice team will review your information and follow up as needed.
